How to order:

There are two steps required to order a test. (please see below) Tests will NOT be shipped until BOTH steps have been completed. DFQS has two proficiency test mailings a year, July and January.  Although the tests may be ordered at any time, proficiency tests will only be mailed during the defined dates as posted on our website.

Due to high demand, we are now shipping our proficiency tests on a quarterly basis.
(All dates are approximate and subject to change)

*Organizations ordering proficiency tests outside the US will share the cost of shipping. DFQS will pay the outbound shipping fee and the buyer will pay the return shipping fee. The schedule for proficiency tests is as follows:

Step 1:

Download a Non-Disclosure Agreement Form (NDA). The NDA form must be signed by a representative of the company/agency.

Step 2:

If you would like your results to be submitted to ASCLD/LAB your quality manager must complete a release form which you can download here or it can be also found on the cd that shipped with the media. Only then will your results be released to a third party.

Submit your test order form
